Family Assistance Program

Are you a CHD patient who is currently hospitalized, or has an an upcoming hospital stay? Requests for Warrior Boxes can be submitted up to one month before scheduled hospitalization. 

For families with a child currently experiencing an extended hospital stay due to CHD or related complications. This grant provides assistance with medical expenses, lodging, food, parking, and other financial burdens that come with weeks or months spent in the hospital.

For families who need to travel for a child’s heart-related surgery, treatment, or specialized care. This grant helps cover transportation, lodging, and other travel-related expenses when care is not available close to home.

For families who have recently received a diagnosis of a critical or complex congenital heart defect. This grant offers support by helping cover early medical expenses, travel, lodging, or and anticipated hospitalization as families begin their medical journey.

For families who have lost a child due to congenital heart disease or a related condition. This grant provides compassionate support to help ease the financial burden of funeral and memorial expenses.
