NO PURCHASE OR DONATION IS NECESSARY TO ENTER OR WIN. A PURCHASE OR DONATION DOES NOT INCREASE YOUR CHANCES OF WINNING.

1. Eligibility: The Hawaiian Vacation Giveaway (the “Giveaway”) is open to all

people who care about Claire the Heart Warrior Memorial Foundation (“Claire the Heart
Warrior”) and helping children and family members who face challenges of Congenital Heart
Disease and Heterotaxy and are 18 years old by October 25, 2025. The Giveaway is void
where prohibited by law. Board members and employees of Claire The Heart Warrior and
their immediate family members and/or those living in the same household are not eligible
to take part. The Giveaway is subject to all applicable federal, state and local laws and
regulations.

2. Agreement to Rules: By participating, you agree to be fully and unconditionally
bound by these Rules, and you represent and warrant you meet the eligibility requirements
described here. You also agree to accept the decisions of Claire The Heart Warrior as final
and binding as they relate to the Giveaway.

3. Giveaway Entry Period: Entries will be accepted starting on June 25, 2025, and
ending October 25, 2025. All entries must be received by October 25, 2025 7:30PM MT.
The Giveaway entry period may be extended beyond the deadline, in which case these Rules
will be updated accordingly.

4. Suggested Donation Amounts: We are grateful for all donations in any
amounts, but no donation is required to win. To support Claire The Heart Warrior and its
mission, we suggest a $20 donation, which will entitle you to one entry ticket; or $50, which
will entitle you to three entry tickets. All donations will be used exclusively for the mission of
Claire The Heart Warrior to help children and their families facing the challenges of
Congenital Heart Disease and Heterotaxy.

5. How to Enter: Tickets can be acquired by completing a form at any Claire The
Heart Warrior event, at the Oakley Rodeo, or the Francis Frontier Days Rodeo, or at Kamas
Valley Fiesta Days; a Claire The Heart Warrior booth will be at all events. You may also
enter by sending your name, address, email address and phone number to this address: Claire
The Heart Warrior, 334 West 2500 North, Kamas, Utah 84036; all mail entries must be
postmarked by October 20, 2025. The entry must fulfill all Giveaway requirements to be
eligible to win. Entries that are not complete or do not follow these Rules may be
disqualified at the sole discretion of Claire The Heart Warrior. You may enter only once, or
as described in paragraph 4 above. You may not enter multiple times by using multiple email
addresses, identities or devices to circumvent the Rules. If you use methods to get around

these Rules, your submission may be removed from eligibility at the sole discretion of Claire The Heart Warrior.

6. Prize: The Giveaway prize winner will receive a 5-day/4-night stay in Kauai,
Hawaii, for two (2) adults, plus two gift cards for food and expenses (approximate retail
value: $5,000; the Giveaway prize value will not exceed $5,000). No cash or other prize
substitution is allowed except at Claire The Heart Warrior’s discretion. Prize-related
expenses, including federal, state, and/or local taxes, are the sole responsibility of the
winner. No substitution of prize or transfer/assignment of prize to others or request for the
cash equivalent by winners is allowed.

7. Odds: The odds of winning depend on the number of eligible entries received.

8. Winner Selection & Notification: The winner will be selected in a random
drawing at the Claire The Heart Warrior benefit dinner at the Red Barn in Oakley, Utah;
the dinner will begin at 6:30pm MT. If the winner is not present at the dinner, the winner
will be notified within five (5) days following the winner selection via email or text message
to the address submitted by the entrant. Claire The Heart Warrior will have no liability for a
winner’s failure to receive notices due to the winners spam, junk e-mail or other security
settings or for the winner’s submission of incorrect or otherwise non-functioning contact
information. If the selected winner cannot be contacted, is ineligible, or fails to claim the
prize within seventy-two (72) hours from the time award notification was sent, the prize may
be forfeited and an alternate winner selected. The receipt by winner of the prize offered in
the Giveaway is conditioned upon compliance with all federal and state laws and regulations.
ANY VIOLATION OF THESE OFFICIAL RULES BY ANY WINNER
(DETERMINED AT CLAIRE THE HEART WARRIOR’S SOLE DISCRETION) WILL
RESULT IN THE WINNER’S DISQUALIFICATION AS WINNER OF THE
GIVEAWAY AND ALL WINNER PRIVILEGES WILL BE IMMEDIATELY
TERMINATED.

9. Terms: Claire The Heart Warrior reserves the right, in its sole discretion, to
cancel, terminate, change or suspend the Giveaway should (in its sole discretion) a virus,
bugs, non-authorized human intervention, fraud or other causes beyond its control corrupt
or affect the administration, security, fairness or proper conduct of the Giveaway. In that
case, Claire The Heart Warrior may select the winner from all eligible entries received before
and/or after (if appropriate) the action taken by Claire The Heart Warrior. Claire The Heart
Warrior reserves the right at its sole discretion to disqualify any individual who tampers or
tries to tamper with the entry process or the operation of the Giveaway or website or
violates these Rules. Claire The Heart Warrior has the right, in its sole discretion, to maintain
the integrity of the Giveaway, to void entries for any reason, including, but not limited to:
multiple entries from the same person (except as permitted under these Rules); or the use of
bots, macros, scripts or other technical means for entering. Any attempt by an entrant to
deliberately damage any web site or undermine the legitimate operation of the Giveaway may

violate criminal and civil laws, and should such an attempt be made, Claire The Heart
Warrior reserves the right to seek damages from any such person to the fullest extent
permitted by law.

10. Limitation of Liability: By entering you agree to release and hold harmless Claire
The Heart Warrior and its affiliates, advertising and promotion agencies, partners,
representatives, agents, successors, assigns, employees, officers and directors from any
liability, illness, injury, death, loss, litigation, claim or damage that may occur, directly or
indirectly, whether caused by negligence or not, from (i) the entrant’s participation in the
Giveaway and/or his/her acceptance, possession, use, or misuse of the prize or any part of
it, (ii) technical failures of any kind, including but not limited to the malfunctioning of any
computer, cable, network, hardware or software; (iii) the unavailability or inaccessibility of
any transmissions or telephone or Internet service; (iv) unauthorized human intervention in
any part of the entry process or the Giveaway; (v) electronic or human error that may occur
in the administration of the Giveaway or the processing of entries.

11. Disputes: THE GIVEAWAY IS GOVERNED BY THE LAWS OF THE
UNITED STATES AND UTAH, WITHOUT RESPECT TO CONFLICT OF LAW
PRINCIPLES. As a condition of taking part in this Giveaway, the entrant agrees that any
disputes that cannot be resolved between the parties and causes of action arising out of or
connected with the Giveaway, shall be resolved individually, without resort to any form of
class action, exclusively before a court in Utah having jurisdiction. In any such dispute, under
no circumstances will the entrant be allowed to obtain awards for, and hereby waives all
rights to claim punitive, incidental, or consequential damages, including reasonable attorneys;
fees, other than entrants’ actual out-of-pocket expenses (i.e. costs associated with entering
the Giveaway), and participant further waives all rights to have damages multiplied or
increased.

your request along with a stamped, self-addressed envelope to: Claire The Heart Warrior,
334 West 2500 North, Kamas, Utah 84036. Requests must be received by December 31,
2025.

13. Sponsor: The sponsor of the Giveaway is Claire The Heart Warrior, a 501(c)(3)
foundation incorporated in Utah, 334 West 2500 North, Kamas, Utah 84036.
